Becoming a member of Premier Patient Care is easy.

Look in your health plan directory for Premier Patient Care. The directory will list all of our physicians, their office locations and the health plan code number that you need to use when selecting a physician.

Use this number on your health plan enrollment form or when enrolling over the telephone to become a member of Premier Patient Care. You may also switch to a Premier Patient Care doctor by contacting your health plan and telling them that you would like to join Premier Patient Care.

Search the online physician directory or call (442) 231-8101 to receive an informational packet about Premier Patient Care, which includes affiliations, rosters, and other helpful information.
